Maher al-Assad commands the Republican Guard and the 4th Armored Division — Syria's most elite, best-equipped, and most politically reliable military units. Unlike regular army units which include Sunni conscripts, the 4th Armored Division and Republican Guard are heavily Alawite, intensively trained, and equipped with the Syrian military's best armor and artillery. When the 2011 uprising began in Daraa, Maher's forces were dispatched first. Tanks entered Daraa on April 25, 2011 — the 4th Division's M1As and T-72s. The same units were used in Homs, Hama, and Aleppo. EU sanctions (May 2011) specifically designated Maher as 'commanding officer responsible for the crackdown on demonstrators in Syria.' US sanctions described his 4th Division as 'the primary driver of violence' against the civilian population. Defectors from the Syrian military described Maher personally making operational decisions during the early crackdown — going beyond what his formal command role required.

When the Syrian uprising began in 2011, Maher al-Assad's 4th Armored Division was deployed in multiple Syrian cities. According to Human Rights Watch and UN Commission of Inquiry reports, the 4th Division participated in the military assault on Deraa in April 2011, the siege of Homs, and subsequent operations across Syria. US and EU sanctions imposed in 2011 described Maher as 'directing military operations' against civilians. Most significantly, US government assessments and British intelligence reports cited by Reuters and the Guardian identified the 4th Division's 155th Brigade as the unit responsible for the August 21, 2013 Ghouta sarin attack — the worst chemical weapons attack since Halabja. Maher has been sanctioned by the US, EU, Canada, and multiple other countries but has never been prosecuted. He remained in Damascus until the regime's fall in December 2024, when he reportedly fled.
